Go to Image>Mode> choose Grayscale. Or when you're making a new document you can choose the Color to be Grayscale(it's on RGB by default), that way every pic you insert in it will automaticaly be grayscaled.

Well depending on what size you would want the boxes...this is what you would do:

New document: 4x4, 100 ppi, transparent background.

Zoom in to 1600%. Get the pencil tool, set it on 1 pixel. Now make it look like this:

Now go to Edit --> Define Pattern.

When you want to use this, go to Layer --> Layer Style --> Pattern Overlay.
